- Payor Mix: 24% Medicaid | 67% PPO | 9% FFS
- Average Production: $3.7M
- 11 equipped operatories
- Doctors: 3 (2 owners and 1 long-term associate)
- RDAs: 4
- RDHs: 2
- Team : 6-treatment planners; 4-assistants; 1-office manager; 6-office staff
- Advanced technology, including digital charts and a Waterlase laser for frenectomies and anesthesia-free dentistry
- Hours: 7:30AM to 4PM – Monday through Thursday

Qualifications:
